Ruka is not a villain, but an obstacle. Mami is the real villain and given how almost everyone hates her in the fandom, I would say she makes a really good job as a villain.

Both would but in different ways. Ruka would be the one to furiously work at the gym and show off to you every gain she has. Chizuru would call you a baka and walk away refusing to have sex, then would one day come to you and strip revealing her ripped abs while blushing and ask you if you are just going to keep watching. As for who would have it easier, I think Chizuru, Ruka's heart conditions likely limits the amount of workout she can do and we know Chizuru has taken self defense training and is very flexible, so she can likely pick up muscle building easier and faster.
