Luffy extends his arm hundreds of feet below to Kaido’s head and grabs on ear, pulling it hundreds of feet above. He talks directly into the stretched out ear.
Luffy: This is fun, right Kaido?
Luffy ties Kaido’s eyes together like shoelaces. Then he skydives downward for a final attack. When the smoke clears, Kaido is on his back in human mode. But Luffy has changed too! The gear’s powers have worn off and the side effect? He is no longer cartoonish and laughing but hyper realistic and depressed.

I don't get why the awakening should have negative effects. Awakening is a natural part of the fruit. It isn't Luffy trying to compound some effect to get stronger or faster at the expense of his body. No other fruit awakening had a negative drawback.
