It depends entirely on the area in question. Some places had better sanitation than others. London was *infamously* filthy to the point where the river Thames was utterly undrinkable, but cities like Paris managed to keep the infrastructure the Romans built working to some degree and were a lot more clean as a result.
Rural areas usually just had outhouses. The notion that people just didn't have an aversion to human waste before 150 years ago was absurd.

The main problem is that some behaviors work for small communities but not large ones.
When your people has been doing their shit in a traditional fashion without much trouble for millennia, and then suddenly you get access to new concepts of hygiene and medicine and your population explodes, all of a sudden your waste disposal "system" is completely inadequate and you are drowning in shit.
But your people have their pride and don't just want to bow to the ways of the "progressives" and foreigners. So people are slow to adapt.
