Yes it's real, and you act like this is a new thing exclusive to this release. The USBD is exactly the same as the JPBD, except with slightly better encoding. Another fun fact, the bluray release of episode 16 is horizontally stretched to fit the 1440x1080 size.

Its probably some bullshit like the rights holder for fly me to the moon wanting an absurd amount of money or denying the license outright. Copyright is a huge pain in the ass especially when you are dealing with two countries with potentially different rights holders. Either way no reason was ever given as to why it was cut so its all speculation.

Legally? The bluray. Torrent? Sephirotic's release, with the 720p Pwnsweet ep 16 in his mega folder. For now.
They lost the master film for this episode, resulting in this shit show. I don't think it's really worth buying the Archives of Evangelion DVD set just for this epsiode.

Money, all we know is that according to Madman who licensed the release down in Australia, it wasn't "financially viable". Likely that the estates of Fly Me to the Moon's songwriters (or whoever controls their publishing these days) wanted a figure that was just too high for how many copies the BDs were predicted to sell.
