I posted some time ago about my pet theory I had for a long time that Ciel was actually a girl, and usurped the Phantomhive seat from the supposed dead 'brother'
It sprung from a rather unique piece of dialog from Liz where she points out Ciels feminine features, and that Ciel wears male heels to appear more manly.
It would also have explained why Ciel was mortified when the twin showed up, because a simple bodily examination would prove who was the *real* inheritor to the Phantomhive estate
You might have read that post of mine a long while ago but I've only posted about it once so It's unlikely. But I'm glad someone else had this idea.

Realistically? Probably not, given this happened some time ago, but there might be some iteration of it, like say the Phantomhive twin being female, and they swapped bodies or something but that'd be twisting things a bit much.
Frankly, there's a lot of things that could be explained in Ciels behavior if he was actually a she.
Ironically, if Sebastian's true form was also female, it would be rather ironic if they were both playing at male.
So I hope something of that sort happens, it'd be a fun twist.
