turns out the reason of the name changing was because reiko got banished from her family, "you are no longer my child, get out from this house, i can no longer let anyone taint ichijouji household anymore", so she got picked up by the shrine and thus was given a new name. basically all that shit happened because of uryuu. so they went back home, and pic related happened (pretty sure you know what that means right??)
