There are a lot of people who think that nagatoro needs a proper love rival instead of wingwomen, or that Shikki should be a rival, but what a lot of people fail to realize is that Shikki doesn’t have to actually be interested in Naoto romantically in order for her to serve as a rival, at least on a narrative level.
Shikki represents everything nagatoro idolizes and wants to be: a beautiful and strong member of the judo club, respected by her peers and not one to give up after failure, as well as a person who is able to speak her mind regarding her opinions of other people (it also helps that Shikki has a more well endowed body, something that nagatoro lacks and is shown to be insecure of).
To nagatoro, especially with the most recent chapter, Shikki represents a threat to her relationship with Naoto because she doesn’t have the “flaws” that nagatoro has, so in her mind, what’s stopping Naoto from simply dating Shikki instead of her? Sure, there was a lot more fan service than usual in the latest chapter, but I think both the visuals and the narrative ill usage are very well these insecurities from nagatoro’s perspective, even though these inner demons are just that: inner. They don’t reflect reality because Naoto has shown repeatedly that he’s only interested in his favorite little noodle.
But Naoto’s feelings are never openly told to her, but rather expressed through actions, hence why Nagatoro isn’t able to let go of these insecurities, which is why Naoto needs to tell her that it doesn’t matter that she might not be the most confident in her abilities, nor the most endowed. What Naoto needs to say is that, despite all of those things, he still thinks of her as beautiful (which would also tie in very well with the manga [pic rel] he was writing in the first chapter. I doubt it’s a coincidence that Elizabeth and Siegfried look like nagatoro and senpai respectively.)

TLDR: shikki is the mental pressure that this series needs in order for these two dorks to finally get together.

Due to her upbringing she's able to activate a pseudo-Byakugan without actually needing to possess said kekkei genkai, but her control over it is imperfect which is reflected by it only appearing in one eye.

Shikki will be the mastermind behind the Orihara revenge arc. After the shower arc she discovered how to force Nagatoro into Total Noodle Mode so she will tell Orihara to hug and flirt with senpai so Nagatoro gets angry with her and beats her. We will have Shikki to thank for the kiss afterwards
