The only thing i can think of is pulling apart the jaws (vetically) or corners of the lips (horizontally) until they tear or getting hit so hard in the mouth that your teeth cut through your cheek. That shouldn't create a clean diamond shape, but I can't think of what would other than deliberately cutting the shape in. Other options that seem unlikely because they would be hard to make symmetrical: getting clipped by a blade or projectile or having someone bite and tear the corner of your mouth.
Takemichi's been bricked in the head by Kisaki. He's also been repeatedly beaten over the back with a bat and punched in the face a lot. As for being ganged up on, you don't need to since Takemichi's whole job it to "inspire" others by getting the shit beaten out of him. His main appeal in story is taking damage and still standing up.
