For me it was the dying mother and the overgrown child. The design of the fat man-baby made me disgusted with the puke and shit. I was also a dumb kid and thought the concept was that he was literally a giant monster baby.
I only later realized after googling the short stories that he was intended to be a mentally retarded man who was part of a circus act.
The lacerated girl wasn't that scary, I do have fond memories of her "death" where her tits bounce before she vanishes.
