I can't think of anyone who could pull him off. Some might look a little like him, but he's more than just an appearance. He has a severity to him that can't really be faked. For example, even if The Rock is big and muscular, he just looks soft. I think a lot of the intensity comes from the eyes. His eyes would need to communicate a lifetime of pain. Look at Klaus Kinski's eyes, for example (even though he obviously doesn't look like Guts). He also has a very particular physique. You can't just cast any old bodybuilder, it wouldn't look right. Finally, the voice is very important. Again, you need to feel the pain in his voice, but also the strength and perseverance. Marc Diraison does the perfect Guts voice in my opinion, so something like him. Putting all of these factors together and you have Guts, but there is no person that has all of these factors in Hollywood. It's have to be some total unknown. Then he has to be able to act.
