not actually sure desu, i think his face looks like this because of the cancer and he basically lost 100kg of weight in a year or so.

He's in a single scene with Maverick, Kilmer's an Admiral and the one that has been helping Mav not to get court martial'd every 5 seconds and also he pulls rank to avoid Mav from retiring. The two meet and have a "conversation" about life and shit. In the movie is said Iceman has cancer and can't speak, so he types on a computer for Maverick to read and so they chat. His character dies shortly thereafter.
