Birthing license should be required, everyone should be on mandatory contraceptive (put in the water source maybe) unless/until they can prove their net worth sufficent for having a child and put that money as a security deposit (in a trust for the child, which can be withdraw every year). A child costs estimately 100k a year bare minimum (2022 value), adjusted for inflation in 18 years the parents need to have at least 3 million.
People with low IQ (therefore can never have 3 million of course) should be sterilized, period.

Health insurance should work in every country more like car insurance. What you pay for car insurance ultimately depends on risk and value. There are a number of things you can do to decrease your car insurance rates such as drive better, drive a certain type of car, drive with a dashcam, and get older. You're obviously free to drive an expensive car with a custom paintjob and drive recklessly, but you have to pay for your own behavior.

Why is health insurance not the same? Why am I, a young, healthy individual forced to subsidize the poor life choices of other people? I eat well, exercise most days, never smoke, sleep 8 hours a night, and have no prescription medication. The amount deducted from my pay for national healthcare should be minuscule compared to others who engage in risk-taking behavior.
