It's not like that, he has expressive aphasia and it's a nurological problem and above all a linguistic problem. 90% of those affected (even the very young) simply have problems formulating and writing complete sentences even though they think regularly. The general confusion arises from the fact that some types of dementia also damage the area of the brain responsible for the formulation of sentences, inducing (broca's) aphasia as a side effect.

Yes, I know aphasia is not necessarily indicating a cognitive deficit.
However, in his case, the etiology is almost certainly some form of neurodegenerative disorder. He has been having issues for years that are getting worse, and his daughter said that he is having cognitive issues.
