he basically killed himself by going top down (!) into a vertical tunnel (!!) that was barely as wide as his body (!!!) that had multiple protruding rock faces that snagged against him (!!!!!) and basically made it so he was like the finger in a chinese finger trap

Caving is very safe if you aren't doing actual exploration. I think part of the appeal is that it still feels dangerous and thrilling, but it isn't.
What happened to this guy was he got confused about where he was and mistook a narrow, almost vertical hole for a passage to the next part of the cave, so he just kind of flung himself into it. It ended up being a dead end, and he was stuck. Pretty senseless and avoidable.
