The way you are supposed to watch the movie is you are supposed to understand that that is the metaphor in use, "holding on too tight = choking". he kills her by wanting to have her too much. And the movie hopes that you'll be wise enough to accept that, well the kids need to actually be born so let's say the choke had a slow acting effect.
