My first shift managing truckers by myself I sent half the night fleet out to random places that weren't open. We failed on roughly 100 loads because of that. The scheduler at the time was really shitty at his job, and did not plan anything. It wasn't my job to plan, but the previous manager had apparently just done it on their own so the scheduler got comfortable in getting covered by someone else.

When I was a teen I worked in a waste management facility.
I forgot to fuck up by not closing a water hose when leaving. It burst from the constant differential water pressure so when I came back next morning everything was flooded and a lot of electrical equipment had to be replaced.

Getting a job in the first place. I worked at a supermarket in the midwest for about a year. Everyone working there was either A. Abusing narcotics B. Mentally handicapped or C. Old. The pay was pretty lousy and there was a shortage of key supplies, i.e. there weren't enough trash bags to go around. We would also run out of the cleaner used to wash the dishes and containers. There was one time when the pipes in the deli counter started leaking and we had to remove all of the goods or risk contamination. My advice for anyone buying deli meat at a grocer is to make sure you can see the deli machines. In many places the machines are not cleaned regularly and lots of gunk inevitably gets mixed in with the product. Also bear in mind that supermarket employees are incredibly miserable people and their souls left the building a long time ago.

Yeah, one of my driver's killed a guy last month because the automatic brakes didn't kick in. He was drunk and walking across a highway at midnight, but still bad. It's a good thing for the company he was probably homeless and completely unidentifiable after.
